The Health Improvement Partnership (HIP) of Santa Cruz County, California, serves as the backbone organization for a coalition of stakeholders: cultivating shared goals, collecting data from stakeholder groups, establishing venues for collaboration, communicating clearly and transparently, and incubating new care models.
