QI Coach - Tower Hamlets Community Health Services

Fatima Hafesji is the Lead Pharmacist for Tower Hamlets Community Health Service and a QI coach

Fatima has worked within ELFT since 2010, initially at the City and Hackney centre for mental health as a specialist clinical Pharmacist, moving to Tower Hamlets CCG as the Lead Pharmacist for Medicines Waste and currently as the Lead Pharmacist for Tower Hamlets Community Health Service in ELFT. She completed Wave 10 of the ILP in 2021 and has since used principles of QI in leading projects and within her day to day work.

Here’s what she had to say about her role and QI:

“QI and medicines seem such a perfect fit! There are numerous areas where QI can be implemented to achieve improvement in processes or patient outcomes. As experts in medicines management and optimisation, pharmacists and pharmacy technicians are ideally positioned to improve current processes and reduce avoidable medicines-related harm; QI approaches are vital to creating lasting change. I applied QI principles to a successful medicines optimisation programme within Tower Hamlets CCG and more recently leading a project on medicines incident reporting within Tower Hamlets CHS. My only area of interest isn’t just medicines though, I find it equally as interesting to learn more about QI implementation in other areas of the healthcare system and this is why I am embarking on the QI coaching journey.”

A QI Coach role involves:

  • Coaching QI teams within directorates, meeting with the team regularly
  • Deeper knowledge of improvement methods and tools

Their responsibilities are:

  • Help engage people and teams in QI
  • Support project teams to develop ideas and strategy, using QI tools, and advise on how to complete project documentation
  • Support project teams in using QI methodology, including PDSA cycles and data over time
  • Provide monthly update on team progress to sponsor
  • Teach and explain use of QI tools and methods
  • Attend supervision with QI lead
