Creating a Change Theory

Triple Aim work will involve developing and managing a portfolio of interventions to help you meet your aims for the population. These interventions can involve a range of change approaches, including QI projects, Community work, service developments and more. A theory of change, described in QI by a driver diagram, can help you make the best use of resources and ensure your interventions reach the right areas.
