We are Tower Hamlets Community Health Services and we are proud to deliver high quality and innovative healthcare to the diverse population of Tower Hamlets. We have taken an active role in the changes to how health and social care in Tower Hamlets are delivered and organised with the establishment of the Tower Hamlets Together initiative with the aim of improving integration, coordination and improving outcomes for all that live in Tower Hamlets.
We believe that good care starts with listening to our patients and their families and so we are planning to use feedback from service users wherever we can in the planning and implementation of QI in our directorate. We are hoping that we can use QI methodology to continually improve the way we work and help us to help our patients live longer, happier and healthier lives.

A HANDY APPROACH – QUICK WAYS OF LEARNING IN TIME LIMITED ENVIRONMENTS
Multiple barriers prevent staff in Community Health Services (CHS) from asking what matters to the people receiving care. A person-centred, safe and easy to use checklist with five questions, called the HANDY APPROACH TO CARE was previously tested successfully in a highly functional team in Newham CHS.
